Informational Memo 25-012 American Rescue Plan Act (ARPA) Home and Community-Based Services (HCBS) Direct Provider Payment
The purpose of this informational memo is to inform select HCBS Providers that, as outlined in last year’s supplemental budget request that was signed into effect on Feb. 27, 2025, the Department of Health Care Policy and Financing (HCPF) is implementing an American Rescue Plan Act (ARPA) Home and Community-Based Services (HCBS) direct provider payment.

Two New Programs of All-Inclusive Care for the Elderly (PACE) Centers Now Open
HCPF is pleased to announce the opening of two Programs of All-Inclusive Care for the Elderly (PACE) centers.
Hospice of Metro Denver Inc, doing business as Colorado PACE is Colorado's newest PACE organization. The PACE center, located at 13650 E. Mississippi Avenue, Aurora, CO 80012, was approved to enroll participants effective 3/17/25. Colorado PACE will serve parts of the following counties: Adams, Arapahoe, Denver, Douglas, and Jefferson. Rocky Mountain PACE was approved to enroll participants at its third PACE center effective 3/27/25. The new center is located at 1420 Quail Lake Loop, Colorado Springs, CO 80906. ✴️ LTSS CCM System Stabilization and Enhancement Dashboard Update
HCPF made significant progress in 2024 to stabilize the LTSS CCM System. As work continues, HCPF acknowledges remaining issues across the CCM System in three key areas including Streamlining Interfaces, Data Quality, and Reporting for Case Management Agencies. Work by HCPF’s contractors (Gainwell and AssureCare) is underway to stabilize the CCM System by remediating these remaining issues. A new integrated schedule to resolve the remaining issues was provided to HCPF in December 2024. In addition, work is underway to complete key initiatives to improve access to care for our members including new programs such as Community First Choice, Money Follows the Person, the new Colorado Single Assessment, as well as a number of other enhancements that were delayed as a result of stabilization issues.
On April 1, a new update related to this important work was published. The next update will occur on April 15 (every other Tuesday).

Direct Care Worker Appreciation Week is April 6-12, 2025
The Direct Care Workforce Collaborative (DCWC) Leadership Team sought a joint resolution in 2024 to declare the first full week of April, every year, as Direct Care Worker Appreciation Week. This year, the dates are April 6-12, 2025. Providers are encouraged to regularly recognize the important work of direct care workers, especially during this dedicated week in April. A Media Kit and Resource Guide has been put together by the DCWC Value & Awareness Action Group to support these efforts, with social media posts, sample messages, ideas for showing appreciation to staff, and resources. These kits include helpful resources for both workers and providers. Contact HCPF_DCWorkforce@state.co.us with any questions.

✴️ Webinar: Update on Office of Community Living (OCL) Priority Projects
Join OCL for updates related to the implementation of the following key priority projects:
- Community First Choice (CFC)
- Children with Complex Health Needs (CwCHN) Waiver
- Colorado Single Assessment and Person-Centered Support Plan (CSA and PCSP)
- Supports Intensity Scale (SIS) Decommissioning
- Nursing Facility Transition and Diversion Efforts
- Skilled Nurse Assessor and Pediatric Long-Term Home Health (PLTHH) Prior Authorization Request (PAR) Restart.

✴️Nurse Assessor Program

Children's Disability Advisory Committee (CDAC)
The CDAC serves in an advisory capacity to HCPF and makes recommendations regarding issues related to benefits for children and youth with disabilities age 20 and younger, provider education interventions and application of standards. All interested stakeholders are welcome.

Skilled Nursing Facility (SNF) Social Work Office Hours with HCPF
This informal meeting will be a time where Social Workers, Social Services Directors, and NHAs at Skilled Nursing Facilities will be able to meet with HCPF to ask questions and provide direct feedback. HCPF will provide updates on our current and future projects to include, but not limited to: Group In-Reach, Individual In-Reach, Community Transitions, Pay for Performance, and Rapid Reintegration. This meeting will be a great time to get clarification on projects that have recently started and plan to begin in 2025.
This informal meeting will be a time where Social Workers, Social Services Directors, and NHAs at Skilled Nursing Facilities will be able to meet with HCPF to ask questions and provide direct feedback.
In these meetings, HCPF will provide updates on our current and future Office of Community Living (OCL) projects to include, but not limited to: Group In-Reach, Individual In-Reach, Community Transitions, Pay for Performance, and Rapid Reintegration. ✴️Private Duty Nursing (PDN) and Long-Term Home Health (LTHH) Policy Team Office Hours
Virtual meetings held for training and technical assistance and will be hosted by HCPF PDN and LTHH policy staff on the second Tuesday of every month. The meetings will be an open forum for questions and information sharing. Stakeholders can submit questions or topics for discussion before the meeting via email to homehealth@state.co.us or submit them through the live queue. All interested stakeholders are welcome.
The meetings will be held the second Tuesday of every month from 1 to 2 p.m. For more information, see the Long-Term Home Health Program page.
