Thank you! - HCPF Annual Stakeholder Webinar Materials Posted

Thank you to the more than 2,400 attendees who joined the Department of Health Care Policy & Financing (HCPF) leaders for our Annual Stakeholder Webinar on August 12.

We discussed emerging federal threats to Medicaid, our processes to tackle them, state budget challenges, Medicaid cost trend drivers, and priorities for fiscal year 2025-26. Materials from the webinar are on our website. We also invite you to bookmark our Understanding the Impact of Federal Changes to Medicaid website where we will share key implementation information and post updated fact sheets as well as opportunities to engage with us further. You can send questions to hcpf_HR1@state.co.us and, if you have not already, please sign up for the At A Glance Newsletter to keep informed. Thank you again for your partnership and engagement as we work together in this chapter of dynamic change.

✴️ At-Risk Office Hours/Work Group Sessions for Regional Accountable Entities (RAEs)

This informal meeting will be a time where RAEs can ask questions and get feedback regarding At-Risk Diversion (the meeting is optional and not required).

About At-Risk Diversion:

At-Risk Members and their chosen supports will receive a targeted outreach to evaluate and/or re-determine service needs, available resources, and potentially any additional supports, services, or referrals to mitigate risks and assist with keeping Members safe in a community setting of their choice. This new process will include targeted outreach to Health First Colorado Member(s) living in the community who are determined to be At-Risk. HCPF will request a generated list of Members with the highest risk for admission to an institutional setting such as a nursing facility based on their risk score and it will include certain characteristics, diagnoses, and/or behavioral needs.

Care Coordinators and any other interested stakeholders can join this session.

The meeting will be held:

Wednesday, September 3, 2025

1:30 to 2:30 p.m.
