✴️ LTSS CCM System Stabilization and Enhancement Dashboard Update
HCPF made significant progress in 2024 to stabilize the LTSS CCM System. As work continues, HCPF acknowledges remaining issues across the CCM System in three key areas including Streamlining Interfaces, Data Quality, and Reporting for Case Management Agencies. Work by HCPF’s contractors (Gainwell and AssureCare) is underway to stabilize the CCM System by remediating these remaining issues. A new integrated schedule to resolve the remaining issues was provided to HCPF in December 2024. In addition, work is underway to complete key initiatives to improve access to care for our members including new programs such as Community First Choice, Money Follows the Person, the new Colorado Single Assessment, as well as a number of other enhancements that were delayed as a result of stabilization issues.
On April 29, a new update related to this important work was published. The next update will occur on May 13 (every other Tuesday).

The purpose of this Operational Memo is to inform Members enrolled in the Children’s Home and Community-Based Services (CHCBS) and Children with Life Limiting Illness (CLLI) waivers, Case Managers, Case Management Agencies (CMAs), and Providers that the CHCBS and CLLI waivers will be merged and retitled as the Children with Complex Health Needs (CwCHN) waiver available starting on July 1, 2025. In addition, this memo will provide operational guidance to CMAs for the upcoming transition.

Working Adults with Disabilities Stakeholder Meeting
The Health First Colorado Buy-In Program for Working Adults with Disabilities allows adults with a qualifying disability "buy into" Health First Colorado (Colorado's Medicaid Program). Members who work and earn too much to qualify for Health First Colorado may qualify for the Buy-In Program. Interested stakeholders are invited to one of two virtual stakeholder meetings to learn about updates being made to the Working Adults with Disabilities Buy-In Program effective July 1, 2025.
At this meeting, staff from the Department of Health Care Policy and Financing (HCPF) will present an overview of the changes being made to how Health First Colorado members enroll into the Buy-In Program for Working Adults with Disabilities. These changes will specifically impact Health First Colorado Members who also receive services through Home and Community-Based Services (HCBS) waivers. Attendees are invited to learn and ask questions about the upcoming changes.

Stakeholder Input on Community Connector Unit and Regulation Changes

The Office of Community Living has tentatively received budgetary authority through R-05 to place a cap on the amount of Community Connector service that can be authorized under the Children’s Extensive Services (CES), and the Children’s Habilitation Residential Program (CHRP) waivers. With this tentative approval, HCPF proposes an annual cap of 2,080 units per Support Plan year, beginning July 1, 2025. This change will take place upon a member’s Continued Stay Review (CSR). The cap on services will not apply to existing Prior Authorization Requests (PARs), meaning a member will not need to reduce or modify already approved services.

Rapid Reintegration Rule Revision Workgroup
This workgroup is for case managers, transition coordinators and other interested stakeholders to collaborate and advise HCPF as it adds regulations for Rapid Reintegration. Rapid Reintegration is the Person-Centered process that occurs when an individual who is seeking admission to a nursing facility, is interested in, or does not oppose, living in the community. This rule revision is necessary to ensure compliance with the DOJ agreement and consistency in providing individuals with information about supports and services in a timely basis.

✴️ Community First Choice (CFC) Council Meeting
The CFC Council is in place to collaborate with and advise HCPF on the design and implementation of a CFC option in Colorado. Primarily made up of individuals with disabilities, those 65 years or older, and their representatives, the council’s experience and expertise will help ensure that CFC is implemented in a way that best meets the needs of Coloradoans with disabilities.

Skilled Nursing Facility (SNF) Social Work Office Hours with HCPF
This informal meeting will be a time where Social Workers, Social Services Directors, and NHAs at Skilled Nursing Facilities will be able to meet with HCPF to ask questions and provide direct feedback. HCPF will provide updates on our current and future projects to include, but not limited to: Group In-Reach, Individual In-Reach, Community Transitions, Pay for Performance, and Rapid Reintegration. This meeting will be a great time to get clarification on projects that have recently started and plan to begin in 2025.
This informal meeting will be a time where Social Workers, Social Services Directors, and NHAs at Skilled Nursing Facilities will be able to meet with HCPF to ask questions and provide direct feedback.
In these meetings, HCPF will provide updates on our current and future Office of Community Living (OCL) projects to include, but not limited to: Group In-Reach, Individual In-Reach, Community Transitions, Pay for Performance, and Rapid Reintegration. Pre-Admission Screening and Resident Review (PASRR) Program: New PASRR Vendor and System Training Series
HCPF is excited to announce we have partnered with new PASRR vendor Maximus. Beginning July 1, 2025, Maximus will be replacing Telligen. To help ensure a smooth transition, please note the following training opportunities. These trainings are directed to Case Management Agencies (CMAs), Hospital Case Managers and discharge planners, nursing facilities (anyone in their facility who works with PASRR), and anyone who is doing PASRR as part of their responsibilities.
Each training session will have the same training content, so it is only necessary to sign up for one training per content area, although you are welcome to attend multiple sessions. All training sessions will be recorded and available to view through the PASRR program page.
These training sessions are also located on the OCL Stakeholder Engagement Page. You may view them there and copy to your individual calendar.
