Submit Provider Enrollment Application via DocuSign!
The Family PACT Provider Application (DHCS 4468) can now be completed and submitted securely via DocuSign. The additional required application forms will be provided to provider applicants via DocuSign upon approval of the DHCS 4468 form.
Coming Soon: PAVE Implementation for
Family PACT
New or actively enrolled eligible provider types will soon be able to enroll, make changes, and disenroll from the Family PACT Program via the Provider Application and Validation for Enrollment (PAVE) portal. Implementation is scheduled for July 2022. Additional details will be announced in a future Family PACT Update.

Virtual/In-Person Family PACT Provider Orientation

LMS Learning Tracks

OFP created LMS learning tracks to help clinic staff identify their role and the required training best suited for their position. 

  • Site Certifier
  • The Medical Director, Physician, Certified Nurse Practitioner or Certified Nurse Midwife who is responsible for overseeing the family planning services at the site.
  • Clinicians
  • Physicians and Non-Physician Medical Practitioners (NMPs) including Nurse Practitioners, Physician Assistants and Certified Nurse Midwives, who are NOT identified as the site certifier.
  • Administrative
  • Medical Assistants (MAs) and front office staff who certify clients for enrollment, deliver client education and counseling and manage medical records on behalf of the Family PACT Program.
