| | CLIA-Waived Tests and Family PACT | | |
What is a CLIA-waived test?
- The Clinical Lab Improvement Amendments (CLIA) is a law requiring any facility performing lab tests on human specimens to be appropriately certified.
- “CLIA-waived” tests, such as urine pregnancy tests, are determined by the FDA to have little risk for error. They can be offered in a laboratory with a CMS CLIA Certificate of Waiver (or equivalent).
- Often, point-of-care (POC) testing refers to CLIA-waived tests, but advances in technology have enabled more complex non-waived testing to be performed at the point of care, so “POC” testing is not an equivalent term to “CLIA-waived” testing.
- New CLIA-waived tests are announced in CMS MLN Matters publications; example: MM14025 - New Waived Tests (5/9/25).

The mandatory virtual provider orientation is a live session conducted over Zoom that provides an overview of comprehensive family planning and resources, program benefits and services, client eligibility, provider responsibilities, and provider compliance. Only site certifiers (physicians, physician assistants, certified nurse practitioners, or certified nurse midwives) who are responsible for overseeing the family planning services rendered may register to attend.

Join us October 15-17 for a FREE three-day training series where providers build skills and confidence in intrauterine device (IUD) and contraceptive implant services.
Choose from IUD & Implant Placement Training on October 15-16 (same training offered twice, with bonus hands-on skills lab) or an Implant-Only Placement Training on October 17.
Participants will earn Continuing Medical Education (CME) credits through expert clinical instruction and hands-on practice designed to expand access to long-acting reversible contraception (LARC) across California.

| | Family PACT Portal: What You Need to Know | | |
What Is the Family PACT Portal?
The Family Planning, Access, Care and Treatment (Family PACT) client enrollment and transaction services transitioned to the Family PACT Portal. This platform is housed in CalHEERS, which is where Medi-Cal and Covered California members apply and renew their coverage. The Family PACT Portal simplifies and streamlines the eligibility process for Family PACT clients. The Family PACT Portal consists of the Client Portal and the Provider Portal.

The Client Portal allows users to create accounts, apply for Family PACT, and manage their enrollment status online.

The Provider Portal enables Family PACT providers to efficiently enroll and recertify clients.
To access the new portal, providers are required to update their site certifier contact information in the Provider Application and Validation for Enrollment (PAVE) Portal to receive their login credentials.
Providers who do not have a current site certifier in PAVE are instructed to immediately email the site certifier's first and last name and email address to Providerservices@dhcs.ca.gov or call (916) 650-0414.

Create a Site Certifier Account
Site certifiers will receive an email titled "Create Your Family PACT Account - Action Needed." The email will include an access code valid for 24 hours to create an account. If the account is not created within that time, providers must contact OFP to obtain a new access code.
The current Health Access Programs (HAP) Client Eligibility System was permanently disabled on February 3, 2025. All client enrollments and transaction changes will now be processed exclusively through the new portal.

Create a Provider Staff Account
Site certifiers can provide access to other provider staff by entering their name and email address into the Site Certifier Family PACT Portal Dashboard, which will email them an access code.
The invited staff must create their account using the link in the invitation email they receive.
Once their account is created, they will be able to enroll, recertify, and update client information.

Training
Providers have access to the following portal trainings in the Online Training Center:
| | Health Access Programs (HAP) Identification | | |
As of February 3, 2025, Health Access Programs (HAP) cards are no longer being issued by the Fiscal Intermediary (FI).
HAP cards are now digitized and can be accessed and printed directly from the user’s Family PACT account via the Family PACT Portal. The HAP Identification Number can be printed or shown electronically on a mobile device when visiting a provider. Providers can print the card for the client, or the client may print it themselves.
Returning Unused HAP Cards
Providers are responsible for returning any unused numbered HAP cards to the FI as soon as possible.
To return unused HAP cards, package the unused numbered HAP cards with a cover letter that includes your provider number or NPI used to order the cards. Mail the package via UPS or a similar service to the following address:

| | FAMILY PACT POLICY UPDATES | | Site Certifier Policy Update | | |
Beginning late October 2025, Senate Bill 1131 will be implemented by updating policy to allow one site certifier to oversee up to 10 service locations for designated clinic types specified in the Provider Enrollment and Responsibilities section of the Policies, Procedures, and Billing Instructions (PPBI) Manual..
Providers designating a site certifier to oversee multiple locations must submit a signed updated Family PACT Provider Agreements (DHCS 4469) and the new Site Certifier for Multiple Service Locations Declaration Form, which DHCS will review for approval. Both documents are not yet available but will be released soon for provider use.
The Family PACT Portal will be updated to reflect these changes. These updates serve as user functionality enhancements, focusing on expanding site certifier and provider staff site location limitations while adding more flexibility when editing or changing accounts.

- J3490-U5: Oral, ulipristal acetate, 30 mg - $37.17
- J3490-U6: Oral, levonorgestrel, 1.5 mg - $6.59
- J3490-U8: Injection, medroxyprogesterone acetate, 150 mg/ml - $32.68
- J7304-U1: Patch, contraceptive, hormone-containing, each - $35.68
- J7304-U2: Patch, contraceptive, hormone-containing, each - $65.35

| | | | Reminder: Providers to Inform Family PACT Clients of HAP ID Activation | |
Family PACT providers are reminded that they are required to inform clients at the time of enrollment that their Health Access Program Identification Number (HAP ID) may be used immediately to access Family PACT services. Clients must also be advised that they may receive services from any enrolled Family PACT provider location statewide, not just the enrolling site.

DHCS is providing this additional, important update to Medi-Cal and Family PACT providers that may be impacted by H.R. 1. Please note that this update supersedes DHCS’ prior News Articles published on July 3, 2025, July 8, 2025, July 11, 2025 and July 31, 2025.

| | | | New Sterilization Consent Form Available in October 2025 | |
Starting on October 16, 2025, DHCS will replace the current Consent for Sterilization form PM 330 to the new Consent for Sterilization DHCS 8649.
Providers are strongly encouraged to transition to DHCS 8649 beginning in October. DHCS will continue to accept the PM 330 through August 31, 2026, to ensure a smooth transition and to avoid claim denials, However, starting on September 1, 2026, the DHCS 8649 will be the only form accepted by Medi-Cal for sterilization procedures. Failure to use DHCS 8649 after September 1, 2026, will result in the claim being denied.
The DHCS 8649 will be available to download from the DHCS Sterilization Materials web page on the DHCS website starting on October 16, 2025.

Beginning July 1, 2025, DHCS will be making modifications to the DETEC application in the Provider Portal for the EWC Program to align with data collection requirements from the National Breast and Cervical Cancer Early Detection Program (NBCCEDP) administered by the U.S. Centers for Disease Control and Prevention (CDC).

The EWC Program now covers self-collection HPV testing for cervical cancer screening using the current CPT® codes for HPV testing. The final recommendations on self-collection HPV testing in a health care setting were published by the Enduring Guidelines Committee, which provided updates to the American Society for Colposcopy and Cervical Pathology (ASCCP) guidelines.
