The Family PACT Portal

Launching January 13, 2025

What Is the Family PACT Portal?

The Family Planning, Access, Care, and Treatment (Family PACT) Portal is designed to simplify and streamline the eligibility process for Family PACT clients. The portal consists of the Client Portal and the Provider Portal.


  • The Client portal is designed to enhance the user experience for applicants and clients by allowing them to easily create accounts, apply for Family PACT, and manage their enrollment status online.
  • The Provider portal enables Family PACT providers to efficiently enroll and recertify clients.


Getting Ready for Go-Live:

Below are the key callouts and actions Providers need to be prepared to act on at go-live on January 13, 2025.

Phase 1: Training

Providers now have access to portal training in the Online Training Center.


Guides and short system demonstration videos will be available soon for reference.


Phase 2: Create Account

Site Certifiers will receive an email on Sunday, January 12, 2025 titled "Create Your Family PACT Account - Action Needed." The email will include an access code valid for 24 hours to create an account. If the account is not created within that time, a new access code will be sent every 24 hours for a limited period.


The current Health Access Programs (HAP) Client Eligibility System will be disabled on February 2, 2025. After this date, all client enrollments and changes will be processed exclusively through the new portal.

Phase 3: Delegating

Once the Site Certifier has set up their account in the Family PACT Portal, they can add a Staff Account to support, manage, and delegate as needed.


Adding new Provider Staff accounts will help with account security and reduce password sharing.

Phase 4: Support

If you have trouble with resetting your account password or have blocked your account due to multiple failed one-time-password (OTP) attempts, contact Covered California Service Center: (800) 906-8521

Upcoming Events

Family PACT Provider Orientation

The Provider Orientation is a live session conducted over Zoom that provides an overview of comprehensive family planning and resources, program benefits and services, client eligibility, provider responsibilities, and provider compliance. Only the site certifier (medical director, physician, physician assistant, certified nurse practitioner, or certified nurse midwife) who is responsible for overseeing the family planning services rendered at the service site should register to attend.

In-Person IUD & Contraceptive Implant Insertion Training in Sacramento, CA

The training is suited both for providers who have never placed intrauterine devices (IUDs) and implants, as well as those who have experience with IUD/implant insertions and removals, but who want to fine-tune their skills. The training includes information about patient method selection, counseling and consent, techniques for the use of instruments during placement, and instruction in the insertion of each type of IUD and contraceptive implant currently available in the U.S. Medical doctors, physicians, physician assistants, certified nurse practitioners, and certified nurse midwives are encouraged to attend.

Family PACT Bulletins

Rebranding Presumptive Eligibility for Pregnant Women

In accordance with Assembly Bill 1481, DHCS will rename the program Presumptive Eligibility for Pregnant Women (PE4PW) to Presumptive Eligibility for Pregnant People (PE4PP).


Updated provider manual sections will be released in a future Medi-Cal Update.


September 2024 Family PACT Bulletin 204

CPT® Codes for Family PACT Updated in Evaluation and Management (E&M) Office Visits

Effective retroactively for dates of service on or after October 1, 2023, the Family PACT Program has updated the policy for Evaluation and Management (E&M) CPT® codes 99204 (new patient) and 99214 (established patient) to allow the billing of these codes for males without restrictions.

Rates Update for Specific Healthcare Common Procedures Coding System (HCPCS) Codes

The DHCS has updated rates for the Healthcare Common Procedure Coding System (HCPCS) codes A4269, J3490, and J7304 for the Family PACT Program for the third quarter of 2024.


These rates are effective for the following contraceptives:

  • Contraceptive vaginal gel ($334.41)
  • Oral, ulipristal acetate, 30 mg ($37.21)
  • Oral, levonorgestrel, 1.5 mg ($6.83)
  • Injection, medroxyprogesterone acetate, 150 mg/ml ($33.84)
  • Patch, contraceptive, hormone-containing, each ($40.19)
  • Patch, contraceptive, hormone-containing, each ($65.32)

Provider Manual Revisions

The DHCS has updated the contraceptive services information to reflect the new 2024 U.S. Medical Eligibility Criteria for Contraceptive Use and the U.S. Selected Practice Recommendations for Contraceptive Use in the Program Standards section of the Policies, Procedures and Billing Instructions (PPBI) manual (page 18).

Rates Update for Specific Contraceptive HCPCS Codes

Effective for dates of service on or after October 1, 2024, the rate for the following HCPCS codes has been updated:

  • A4269 with modifier U5 (contraceptive vaginal gel, Phexxi® [lactic acid, citric acid, and potassium bitartrate])
  • J7304 with modifier U1 (patch, contraceptive, hormone-containing, each; Xulane)
  • J7304 with modifier U2 (patch, contraceptive, hormone- containing, each; Twirla)
  • J3490 with modifier U5 (oral, ulipristal acetate, 30 mg)
  • J3490 with modifier U6 (oral, levonorgestrel, 1.5 mg)
  • J3490 with modifier U8 (injection, medroxyprogesterone acetate, 150 mg/ml)

Paragard Intrauterine Copper Contraceptive Single-Hand Inserter

The DHCS is notifying providers that a single-hand inserter is now available for use with Paragard (intrauterine copper contraceptive), resulting in a new National Drug Code (NDC). Effective for dates of service on or after August 28, 2024, providers are to bill for HCPCS code J7300 with the appropriate NDC:

  • Original NDC: 59365-5128-01
  • Single-Handed Inserter NDC: 59365-5129-01


Refer to the training videos regarding the insertion and removal of the Paragard intrauterine contraceptive.

Medi-Cal Bulletins

Updated Licensed Midwives Evaluation and Management Codes

Effective for dates of service on or after October 1, 2024, the following E&M codes will no longer be associated with Licensed Midwives (LMs): CPT® codes 99401, 99402, 99403, 99404, 99408, 99409, 99411, 99412, 99450, 99455, 99456, 99473 and 99474.


The E&M CPT codes 99204 and 99214 will become Medi-Cal benefits for LMs.


Updated provider manual sections will be released in a future Medi-Cal Update.
