As Ohio’s cardiovascular practices, clinicians, and patients slowly and steadily transition through this unprecedented pandemic, the Ohio-ACC is working on behalf of our members to create a “new normal“ of cardiovascular care in Ohio.

Ohio-ACC Virtual Spring Summit
Thank you to all of the participants and faculty of the April 10th Ohio-ACC Virtual Spring Summit, which had record participation. Topics included Update on Medical Treatment Strategies in Heart Failure, What's Hot in EP, and Possibilities & Controversies in IC. We were honored to have Deepak Bhatt, MD, FACC deliver the Carl J. Wiggers Memorial Lecture entitled Residual Cardiovascular Risk in Statin-Treated Patients with Elevated Triglycerides: Now We Can REDUCE-IT. Special thanks to our course co-directors Joseph F. Collaborative Maintenance Pathway
For those of you looking for an alternative to the traditional 10-year board certification exam, please consider the ACC’s new product, the Collaborative Maintenance Pathway (CMP). Advocacy
Despite the challenges of the pandemic, the advocacy teams at both the national and state levels have worked hard to advocate for the cardiovascular health of our patients and the well-being of our providers and practices. As an example, the ACC advocacy team was instrumental in the reintroduction in Congress of legislation to minimize the preauthorization burden on seniors with Medicare Advantage.

Finally, congratulations to Ohio’s fellows in training (FITs), who are graduating this month and entering practice. We have put together tools to support your transition to the ultimate level of cardiovascular recognition, Fellowship of the ACC (FACC). Get the details on the FIT to FACC pathway.

As our Board of Trustees election approaches, we would like to ask for your involvement.

The board is seeking nominations for the following vacancies by June 25:
  1. Toledo: Physician nominee from practicing community (which includes a private practice, or full-time staff of a satellite hospital, or full-time staff of a stand-alone hospital system).
  2. Active Duty Military: Physician nominee who is active duty military based in Ohio.

A nominee must be a Fellow, Associate Fellow, Affiliate, Distinguished Fellow, or Honorary Fellow of the American College of Cardiology in Ohio.

Responsibilities:
Here is the full description of the Ohio-ACC Board of Trustees Role. Primarily we are seeking your counsel and service as we work to build a strong voice for cardiology in Ohio. The time commitment is reasonable.

Call for e-Poster Judges



Our annual Ohio-ACC Poster Competition will continue in a virtual format this year! We are looking for interested judges to help make this an exceptional event.

Your role would include:
1. Reviewing the submitted abstracts in early September via an online judging system that is double-blinded so that you will not be judging your own institution's fellows.
2. You will be asked to judge the oral presentation to determine the winner.

I would like to introduce myself as the new Cardiovascular Team Chapter Representative. I have practiced at OhioHealth’s Riverside Methodist Hospital for the past 7 years as an advanced practice provider. My clinical background is in Electrophysiology working on the inpatient service, serving as the advanced practice clinic lead for the Atrial Fibrillation clinic as well as seeing patients in the left atrial appendage closure clinic. I have been involved with Ohio-ACC for the past several years as a CV team committee member and am looking forward to this new role as the CV team chapter representative. I am so happy to be working with all of you and look forward to lots of positive interactions!

Initiatives to Improve the Prior Authorization Process

Collaboration to Improve Prior Authorization
The burden of prior authorizations (PAs) on healthcare delivery and patient access is likely well known to the members of the Ohio-ACC. The growing impact of prior authorizations has become a focus for groups such as the American College of Cardiology, the American Medical Association, and the American Heart Association. The AHA launched a Value in Healthcare Initiative to work on four priority areas to improve cardiovascular care delivery: value-based models, partnering with regulators, predict and prevent, and prior authorization. Some of the solutions proposed, like value-based contracts which relax or bundle PAs, or a shift in utilization management from a payer to a health system, are novel but may require active participation from both payers and prescribers1.

A meeting organized by the Academy of Managed Care Pharmacy to discuss the PA process for PCSK9 inhibitors showed the importance of these communications between payers and providers2. The AMCP surveyed payers and providers before and after the meeting. After the meeting, payers decreased in their belief their PA criteria accurately distinguished patients (72%-50%), became more agreeable to less stringent PA criteria (33%-72%), and recognized the need for standardized processes (50%-83%). Over 80% of participants, both in payer and provider groups, supported programs to educate staff on PA processes.

Expanding Electronic PA Processes
One solution that has gained more visibility in the last few years has been the electronic prior authorization process. Despite the increased implementation of electronic prior authorization processes, a 2020 report by CoverMyMeds revealed that 49% of practices still utilize non-electronic methods of submitting prior authorizations3. Data suggests a prospectively initiated (i.e. in-office) electronic prior authorization can substantially improve time to medication access, on average 13.2 days sooner. Unfortunately, CoverMyMeds found only 17% initiate a PA at the point of prescribing. The challenge facing clinicians is balancing the administrative burden of initiating the proactive PA with their patient care responsibilities.

The Center for Medicare and Medicaid Services (CMS) finalized a new rule at the beginning of 2021 to improve upon the electronic prior authorization (PA) process4. One of the more notable changes highlighted by CMS was a requirement for payers to establish a standardized system for sharing health data between payer-to-provider and payer-to-payer. Another notable change was a requirement to reduce the PA decision time to a maximum of 3 days for urgent requests and a maximum of 7 days for non-urgent requests. Unfortunately, these rules only apply to Medicaid and not Medicare Advantage plans, which limits the impact of these rules. These rules are a step forward, but there is still a considerable need to lower barriers, improve transparency on decisions, and improve patient access that solutions proposed by the AHA Value in Healthcare Initiative may address.
