Our online medical appointment request form streamlines the process for our members,

offering a convenient and efficient way to schedule appointments!

Below are the steps to make an

Online Medical Appointment Request

• Click on the following link to submit your Appointment request with our Participating Providers in Manila: https://appointment.calvos.net

• Enter your name, date of birth and SelectCare Member ID exactly as listed on your Member ID card

• Enter your email and contact numbers

• Enter your preferred appointment date(s) & travel dates 

• Select your preferred hospital facility in the pull down menu

• If you are requesting for an Annual Preventive Exam, check the respective box

• For other medication appointments, complete the information in up to five (5) fields such as physician’s name, type of specialty or purpose of visit.

• Provide information on special requests or unavailable dates in the Notes field

• Upload your referral and medical records. Failure to provide these documents may result in a delay or denial of your request.

• Check mark the reCAPTCHA box for added security and click “Send Appointment Request” to submit your request

• You will receive an email validating your request

• You will receive an email confirming your appointment details within five (5) business days of your request.

IMPORTANT: 

Appointment requests submitted through this method should be new. If you are already in contact with our office and coordinating your appointment, please do not use this system, as it may cause confusion with your ongoing request. If you would like to revise your current appointment schedule, please contact our office to make this arrangement.
