Open Enrollment is the only time each year that you can:

Enroll in the Health Savings Account, for those enrolled or changing their plan type in the High Deductible plan only.

Please remember: If you miss the deadline you will not be able to enroll or decline benefits until next Open Enrollment without a qualifying life event. Please contact our ISU HR Benefits Coordinator, Janice Romero at (208) 282-3106 or [email protected] for more information.
If you DO NOT wish to make any of the changes listed above or enroll in flexible spending, there is no action you need to take during open enrollment.
All forms must be RECEIVED by the ISU HR Office no later than 3 pm on Friday, May 12. Forms submitted past this deadline will not be accepted by the State’s Office of Group Insurance.

FSA Enrollment Form - for enrolling/re-enrolling in either a Medical or Dependent Care Flexible Spending Account (FSA); and Limited Health Care FSA (LHCFSA)

HSA Enrollment Form- for enrolling in the Health Savings Account

NCPERS Enrollment Form- for enrolling in the NCPERS Life Insurance.

Choice Card Form - used for changing medical plan options, changing premiums from pre-tax to post-tax, and/or adding dependents to dental plans. Please note that premiums are automatically set up as pre-tax, unless otherwise discussed.

Medical Enrollment Application - for enrolling or disenrolling dependents on the medical or dental plan. This form can be turned into the HR Office or uploaded via this secure Box link.
