Open Enrollment Begins Today
As previously communicated, the open enrollment period for benefit-eligible faculty and staff will take place starting today (Monday, April 26) through Friday, May 14, 2021.
 
Benefits changes made during this open enrollment period are effective for the FY22 benefits plan year (July 1, 2021 through June 30, 2022).

How to Access and Submit Forms
To support our employees in completing these forms easily and securely, the State of Idaho’s Office of Group Insurance (OGI) and ISU have developed enrollment forms that can be completed and submitted electronically through DocuSign.
 
To access open enrollment forms, step-by-step completion instructions, and for more information about benefits options, please visit https://isu.edu/hr/open-enrollment/. 
 
If you would like to access the forms directly, use the links below:
FSA Enrollment Form - for enrolling/re-enrolling in FSA; and
Choice Card Form - used for changing medical plan options, changing premiums from pre-tax to post-tax, and/or adding dependents to dental plans. 
 
Reminders
All forms must be RECEIVED by the ISU Human Resources Office no later than 3 p.m. on Friday, May 14. Forms submitted past this deadline will not be accepted by the State’s Office of Group Insurance. 
 
Please remember that no action will be necessary if you DO NOT wish to make any of the changes above or enroll/re-enroll in an FSA. Your benefit coverage will continue as-is into the next plan year. FSAs are only in effect for the current plan year. If you want to sign up for a new FSA for next year (FY2022), you must complete a new form.
