Sent-Important Letters to Active Patients

This week please open your letter from Dr. Kordonowy!

Effective January 1,2018 Dr. Kordonowy will no longer be a Medicare Provider Participant. He is going to a Direct Primary Care Membership model for all primary care patients. Patients who see Dr. Kordonowy for Primary Care will need to choose a membership option. Letters packets were mailed this weekend - please open these letters for detailed information. Contract information and practice forms will need your attention. Your response is needed. A stamped return envelope is included in your mailing.

Dr. Kordonowy will be available Fridays from 1-2 PM in his office to answer any questions.

Dr. Kordonowy will be mailing his notice letter to Medicare by mid November. He must have documented full disclosure to Medicare patients as part of opting out of Medicare regulations.
