How To Know If You're Eligible:


OPTION 1: Call the number on your insurance card and ask if you have Renew Active, One Pass or Aaptiv eligibility. If "yes", ask for your "code". That is all we need! Your code will start with an "A", "S", "B" or "P" and will be followed by 9 digits.


Once you've obtained your code, please (1) email the code to carrollton@osteostrong.me AND (2) send a text to

(469)405-1448 letting us know an email has been sent.


OPTION 2: We are also happy to check for you. Please click HERE to fill out our digital form. Once this form is completed, there's nothing more for you to do. We will respond by email within 5 business days.


OPTION 3: If you do not want to use the digital form, you may also send the following information to carrollton@osteostrong.me and we will check your eligibility for you.


  1. Full Name (exactly as it reads on your insurance card)
  2. Membership ID number
  3. Group number (if it's listed on card)
  4. Birth date
  5. Zip Code

We will not be able to do this at your weekly appointment as it is a VERY detailed process, and we do not wish to cause delays in our daily appointment "flow".

  • Not all UHC plans have Renew Active/One Pass/Aaptiv benefits
  • Optum Pricing discounts are based on the CURRENT average OsteoStrong membership price.
  • Once we verify coverage, we'll notify you by email and review "Next Steps".
  • For members with coverage, out-of-pocket membership expense could drop by 33% or more!
  • The BEST news is this is just the beginning! It is quite likely that in the near(ish) future, more insurance companies and more plans will offer this type of wellness coverage. We'll keep you updated on any developments on that front!
