Your Child(rens) Portal Has Missing Forms

One or more of the required Student Forms is missing from your child(rens) file. Please log into your protal to see which form or forms from the list below are missing. By law, your child may not begin school without a complete file of this vital information. Please upload now

These six forms are required for every child: Confidential Student Information and Emergency Contact Form Parent Agreement Form (one form per family) COVID Waiver (one form per family) DC Authorization for Child's Emergency Medical Treatment DC Travel and Activity Authorization Please check the box that allows your child play outside the fenced area so that your class may take a nature walk around the block. DC Universal Health Certificate This form requires the signature of your doctor and "Date of Health Exam" must be August 1, 2020 or later. It will need to be updated if it expires during the school year. This form is required for children age 3 and older on Sept. 1 DC Oral Health Assessment Form This form requires the signature of your dentist and "Date of Exam" must be August 1, 2020 or later. This form will need to be updated if it expires or becomes applicable during the school year. This form is only required for students who require medication or treatment at school. DC Medication and Medical Procedure Treatment Plan This form requires the signature of your doctor. If we do not have this form on file, it is assumed your child does not require any medicine or treatment while at school. This form is only required for students with a diagnosed allergy. Action Plan for Anaphylaxis This form requires the signature of your doctor. If we do not have this form on file, it is assumed your child is allergy free and requires no special care.

Parent Portal Forms Upload Procedure

Create individual pdf files of each of your forms separately. Please create 1 pdf file per form. You will not be able to upload a file that is greater than 1MB. You may want to use an app like Genius Scan. This app allows you to see the resolution (filesize) when you export your file to ensure that it not over the 1MB limit.

Upload your forms into your Parent Portal. Once you have logged into your account, navigate to the Child Info section to upload your forms. Since the Parent Agreement Form and the COVID Waiver only require one copy per family, please attach those forms to your oldest child's section.
