Programs of All Inclusive Care for the Elderly (PACE) is a Medicare/Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility.
Through a strategic partnership between Aging & In-Home Services and Parkview Health, PACE of Northeast Indiana offers both clinical care and social services in one setting.
The PACE benefit is available to anyone who meets the four eligibility requirements:

Hospital2Home
Dementia Specialized Care Transitions

Hospital2Home is a grant program funded by the Administration for Community Living for individuals with Alzheimer’s Disease and related Dementias and their caregivers. The 30-day program provides assistance to bridge gaps in care, reduce caregiver burden and provide resources to keep the person with Dementia at home and out of hospital settings. | |

Thursday, June 15th was World Elder Abuse Awareness Day. About one in ten Americans over the age of 55 has experienced elder abuse.
Elder abuse takes many forms and can happen when an older person is vulnerable. By learning the signs, you can help identify and stop elder abuse.
