I am writing to you with an update about our Community Assistance Program (CAP) which addresses health-related social needs for patients and Medicaid members across Oregon. You are part of a health system or community partnership that collaborates to make this work happen.


This work supports local healthcare providers, hospitals, payers, and CCOs as they access the resources needed to help patients recover, maintain, and improve their health. Our staff prioritize supporting local businesses and community-based organizations who provide goods and services, streamlining payment and facilitating communication with major buyers.


Each year, CAP connects more than 12,500 individuals to critical services such as hotels and short-term rental support; transportation; meal and food delivery; air conditioners, fans, and utility support; and much more.



Thank you to our program partners who collaborate to find streamlined solutions to help health care providers better access services to improve community health.

Let's get together! CCO Operations Meeting and CAP Office Hours 


The first CCO Operations meeting is September 14th at 1pm! To help improve efficiency and improve our communication with partners, PANOW is kicking off a new monthly meeting structure that will start this month. We look forward to seeing you there!

  • Space to review program processes, reports, and structures
  • Opportunity to improve workflows across organizations
  • Opportunity to discuss Track Via challenges and opportunities for improvement
  • Opportunities to address recurring challenges collaboratively
  • Opportunity for PANOW staff to provide updates across organizations (staffing, queues, closures, etc.)

  • Opportunity to ask questions or get clarification
  • Chance to talk one on one with PANOW staff about a specific challenge/data point/TrackVia issue

During our team check-ins each week, we regularly share a “mission moment”, a success story of what we did, and how that made a difference in someone else’s life. Here is one of our mission moments we’d like to share:


Community Partner Feature


Community Warehouse provides donated furniture and household items to neighbors seeking the comfort and dignity of a furnished home while overcoming adversity. Serving the Portland area for more than 20 years now, they recently opened a new donation center in Gresham in partnership with ReClaim It! and Metro. There are 3 locations: Portland, Tualatin, and now Gresham.


Many people in our community are striving to get back on their feet and create solid home foundations for themselves and their children. And yet, they can barely make ends meet. For these families, household furnishings can make a world of difference, and local furniture banks are the solution. There are many ways we can help (including donating safe, clean, and functional items). Check out their website for more information on what they accept, how to support, and share this information with other partners and other contacts. It takes a village to help our community, let your generosity shine!
