Hello Heart is partnering with PEBA to provide you with a free, personal heart health monitoring kit. Your kit will include a free blood pressure monitor that connects to an easy-to-use smartphone app.

The Hello Heart app can help you regularly monitor, understand and manage your heart health—all from the privacy of your phone. The program is available at no cost to State Health Plan primary members (ages 18 and older) who have blood pressure readings of 130/80 mmHg or above or who take blood pressure medication.

Each eligible State Health Plan primary should enroll separately to receive their own blood pressure monitor and tracking app.
