You may be entitled to change your election if you, your spouse, or dependents experience one of the Qualifying Events below. Please notify PPAZ right away if you have a Qualifying Life Event by emailing payroll@ppaz.org
Divorce or legal separation
Open Enrollment for Spouse's employer
Change in number of dependents
Change in employment status that causes loss of eligibility
Dependent satisfies or ceases to satisfy eligibility requirements
Change in coverage under another employer plan
Loss of coverage from government or educational institution
COBRA qualifying event (termination/reduction of hours, employee death, divorce/legal separation, ceasing to be dependent)
Other changes resulting from a judgment, decree, or order; Medicare or Medicaid entitlement
Elections MUST be made within 30 days of your qualifying event
Proof of Qualifying Event will be requested to process your enrollment.
