PSPS Functional Exercise

If you do not have a member agreement on file, please complete the Member Agreement Packet (no member fees/dues required) which includes the following:

• the Member Agreement Form;

• one Site Profile per each site listed in the Member Agreement Form; AND

• at least one VCHCC Email Distribution Registration Form for the organization listed above.
• While one is the minimum requirement, there is no limit to the number of personnel from your organization who can register to receive VCHCC emails. We encourage you to have all appropriate personnel register to receive VCHCC emails so that they may receive important information.

If you already have a member agreement on file, review your site profile and your member agreement form for accuracy of current information included (e.g., facility name, representative and alternate serving this role, contact information, evacuation locations, generator, transportation information, etc.).



If any information is outdated:

• submit an updated Member Agreement Form AND
• submit an updated Site Profile.



If all information in both site profile and member agreement is still current:

• reply to this email advising the VCHCC that all information in your member agreement packet is up to date. Be sure to include the facility site(s) you represent.
