School Dental Clinic – Parent Information Letter
Dear Parent(s)/Guardian(s),
Southland School is partnering with Children’s Dental Health Services (CDHS) to provide an on-site dental clinic for students during the school day.
Dental Clinic Date: Wednesday, February 19, 2026
Location: Southland School
Licensed dental professionals will provide preventive dental services, which may include a dental exam, teeth cleaning, fluoride treatment, and sealants if appropriate. These services help support your child’s overall health and academic success.
What You Need to Do
Please complete and return the attached consent form by Friday, February 13th if you would like your child to participate in the dental clinic. A signed consent form is required for your child to receive services.
The consent form may be printed and returned to school, completed electronically and emailed (aobyrne@isd500.k12.mn.us), or picked up at the front desk. If the consent form is not returned, your child will not be seen.
Cost & Insurance
CDHS accepts Medical Assistance (MA) and MinnesotaCare. Services will be provided at no cost to families. CDHS can also assist families who do not currently have dental insurance.
