Are you submitting mileage claims? You need a Mileage Log

If you’re submitting mileage claims, you need to keep a Mileage Log to support your claims. There’s a downloadable Mileage Log template at passportfunding.ca/resources that can help you keep track of your mileage activity.

Are you claiming airfare transportation?

A pre-approval request (PAR) is required for all airfare expenses, including for Support Worker travel. The PAR application must be submitted to your local Passport agency before you book your travel per the Passport guidelines.

What can your local Passport agency do for you?

After you get Passport funding, your local Passport agency is your first point of contact for help and support. Passport agencies can:

  • Support recipients and their families through the Passport program onboarding process.
  • Help recipients and their families understand the Passport guidelines to clarify the goals of the program and identify eligible and meaningful ways to use funding.
  • Respond to inquiries about invoice submissions and processing
  • Provide a direct point of contact through your Passport Coordinator to best respond to all inquiries via phone, email and/or in-person
  • Encourage recipients and their families to provide accurate information on all forms, claims, invoices, and receipts to ensure continued access to Passport funding and avoid any potential issues.
  • Receive and process information from Developmental Services Ontario (DSO)
  • Listen to and use feedback from recipients and families to improve the program

If you need support, reach out to your local Passport agency – we're here to help!

My son is having a wonderful time with the developmental services of Leeds and Granville and all his volunteering jobs he has. He’s very happy. Thank you for the funding.

- Elaine (Mom), Leeds and Grenville

Diane has been a recipient of Passport funding for many years, and it has been of such help. It helped with her workshop funding years ago in Kitchener and now, in Kingston, Ontario, it helped pay for her attendance at Extend-A-Family until she went into long-term care, at which point we began using the funding for a companion.

We use the Passport funding to provide her with a caregiver who helps with [activities of daily living] and provides amazing companionship, challenging Diane to do puzzles, word searches, colour, sing and dance (seated)—things that she used to love doing. Laurie makes Diane’s days joyful and full of love with companionship and care.

Without Passport funding, we would not be able to do this for Di. We so appreciate this help with her care and the joy it has added to her life. Thank you immensely.

- Loretta (Diane's sister), Kingston

Passport funding has helped me in a major way throughout my daily life. [It has given me more financial support and has allowed me to be even more independent in the community with supporting the cost of presto pass for 12 months. [It] has also supported me… with having a support worker to work close with me… with activities in the community gym [and] daily support as a transgender woman. [It] has given me more freedom and independence as well with using Passport funding on daily life activities. I’ve learned newer skills around how to budget my own Passport funds by myself without help and [this] gave me the skills of budgeting, literacy and knowledge.

Ever since I’ve started Passport funding, I’ve been more involved in the community and it gave me the support to be independent, made me more connected to community, friends, programs and much more. Passport funding also helped me with purchasing arts and crafts materials which I use a lot to keep busy and use to decrease sad moments of my life. It also allowed me to gain more creative skills with my talents.

Passport team members have been so supportive [which] gave me confidence and [made me] feel more connected as well. I’m thankful and grateful for Passport funding, it helped me a lot with financial freedom and independence. Thanks Passport funding.
