You down with TTT?

Orthofix’s TrueLok™ Elevate Transverse Bone Transport (TTT) System is an innovative limb preservation platform designed to support the treatment of complex diabetic foot wounds, chronic limb-threatening ischemia (CLTI), and other severe lower extremity soft tissue defects.

Built around the principles of tibial cortex transverse transport (TTT), a biologic technique derived from Ilizarov concepts, the system utilizes controlled distraction to stimulate angiogenesis and improve distal perfusion. Published clinical literature has demonstrated encouraging outcomes in wound healing and limb salvage among appropriately selected patients.

The TrueLok Elevate System is the first commercially available U.S. platform specifically designed for TTT procedures and integrates with the established TrueLok external fixation family. The platform was developed to provide surgeons with a reproducible, streamlined approach to transverse bone transport while maintaining stability and ease of adjustment throughout treatment.

Key Benefits of the TrueLok Elevate System

Supports Biologic Limb Salvage

  • Designed to promote vascular regeneration and improve microcirculation through distraction-induced angiogenesis
  • Provides an additional treatment consideration for patients with ischemic diabetic foot ulcers and chronic wounds
  • Expands options for complex limb preservation strategies

Streamlined Surgical Workflow

  • Dedicated instrumentation and corticotomy templates help standardize the procedure
  • Modular design allows standalone or hybrid frame configurations
  • Compatible with TrueLok™, TL-HEX™, and TrueLok EVO systems

Precision and Stability

  • Controlled transport mechanism allows accurate, reproducible bone movement
  • Stable fixation minimizes unwanted rotation or loosening during treatment
  • Simple “pull-and-turn” adjustment mechanism enhances usability for both surgeon and patient

Patient-Centered Design

  • Lightweight construct supports patient comfort
  • Simplified adjustment process may improve compliance during distraction and compression phases
  • Frame configuration allows easier access for pin-site and wound care management

TTT continues to gain global attention as a biologic approach to limb salvage, particularly in patients facing high amputation risk due to diabetic foot disease and ischemia. While the technique remains relatively new, growing clinical experience and published data are driving increased interest in its role within comprehensive limb preservation programs.

The addition of the TrueLok Elevate platform strengthens the Orthofix limb preservation portfolio by offering surgeons a dedicated solution that goes beyond traditional fixation, focusing instead on restoring perfusion and supporting long-term limb salvage outcomes.
