PATIENT SAFETY GOALS

ACHIEVING THE "PATIENT SAFETY GOALS"

Identify patients correctly

Use two patient identifiers

Prevent Infection

Handwashing prevents the spread of infection

Improve staff communication

Ensure timely follow-up with patients (ex: test results, medication refills, etc.)

Identify patient safety risks

Using checklists provided is one of the most effective tools to ensure patient safety

Use medicines safely

Double-check to ensure the correct patient is receiving the accurate medication

Use emergency codes correctly

Paging the correct code not only saves time but will also saves lives

Remember, Patient safety is the prevention of avoidable errors and the harm they cause to patients; it is the foundation of good patient care. 



Achieving safety in patient care is part of achieving quality patient care.

In our unwavering commitment to patient safety, the Safety & Risk Management Team embraces the three fundamental principles encapsulated in the three C's of safety —Check, Care, and Communicate.


Check – Stay vigilant to identify potential safety concerns. Regularly inspect your surroundings for any risks that may arise.


Care – Actively contribute to maintaining a secure environment. If you spot safety hazards, such as loose cords or obstructions to emergency exits, demonstrate your commitment to safety by promptly submitting a ticket to the relevant department, whether it's Building Operations or the Information Services Department.


Communicate - In emergencies, time is crucial. Dial 911 immediately when faced with an urgent medical issue. 


Your dedication to the three C's plays a pivotal role in upholding a secure environment for everyone under our care.
