2023 August Edition
President's Message
August is here, and many of our children (including mine) are already back in school! While the return to school is always hectic, I am deeply grateful that COVID-19 remains under control and our children are attending school in person.
|
|
That being said, there has been a slight uptick in hospitalizations, and COVID-19 has been in the news this week. Citing the need to reduce school absences and support child mental health, Los Angeles Unified School District (LAUSD) Superintendent Alberto Carvalho recently stated that children should return to school when “mildly sick – no fever, just maybe the sniffles.”
In a public message to LAUSD parents, Chief Medical Director Dr. Smita Malhotra confirmed this policy and further specified: “If your child has a mild runny nose or cold symptoms that are not bothering them, and they test negative for COVID-19, send them to school. Your child can wear a mask at school when they have these mild symptoms.”
As we head into fall, there will likely be questions about the role of testing and masking in this more permissive COVID-19 policy, not to mention the variations across the many school districts and private schools in southern California. The Chapter 2 School Health Committee remains a useful resource to help answer questions coming from parents and educators as we return to pre-pandemic norms.
Last winter, as COVID-19 was receding, our youngest patients faced a particularly difficult RSV season. Fortunately, pediatricians will have a new tool to help protect infants this winter. The American Academy of Pediatrics just released recommendations for the use of nirsevimab, a new monoclonal antibody to prevent RSV infection. The AAP is recommending nirsevimab for all infants under 8 months born during or entering their first RSV season. As nirsevimab rolls out, please let the chapter know about your experiences obtaining it for your patients.
Finally, I attended the AAP Annual Leadership Conference earlier this month together with Drs. Ruiz, Goldstein, Torices, and other chapter leaders. At the conference, we met with our colleagues from across California (District IX) to share ideas for new initiatives to benefit children, families, and the pediatricians who care for them. We also voted on resolutions which are submitted by AAP members and serve to guide the national AAP board on advocacy priorities for the coming year. I would like to recognize and thank former Chapter 2 president Dr Paula Whiteman for submitting two of the resolutions that were voted on by conference attendees as the top 10 resolutions for the year. As you can see in the article below, the resolutions cover a wide variety of issues including protecting transgender children and their pediatricians, promoting gun safety, and supporting pediatric payments, telehealth, and medical education.
This month’s member spotlight features Dr. Christina Casas of Daisy Pediatrics. I would also like to welcome our new chapter members and encourage them to get involved by joining a chapter committee. Information about our committees and other resources are available on our website, aapca2.org. Your support makes the work of this chapter possible!
Upcoming events in this month’s newsletter include:
- August 28 | AAP Project Firstline Media Training
- August 31 | Connecting with Nature to Mitigate Toxic Stress (Training)
-
September 7 | Rx for Gun Safety Webinar (Courtesy of AAP-OC)
- September 22 | Permanente Medicine Pediatric Symposium
- October 4 | Cardiac Evaluation for Sports Clearance

Best wishes,
Grant Christman, MD, MACM, FAAP

Protect Our Trans and Gender Diverse Youth
Esther H Kang, MD, FAAP
|
|
I met JJ and their mother for the first time during Pride Month. I remember this because of their fingernails. Painted in different swirls of light blue and pink, their nails were reminiscent of the transgender pride flag.
“I got them done for Pride.” JJ happily fanned out their hands and beamed up at me.
JJ, newly 18 years old, was transitioning care to our Med-Peds clinic.

August Update
Immunization Awareness
-
August is National Immunization Awareness Month. Help us spread the word: children’s access to routine vaccination reduces the impact of health disparities and benefits their long-term wellness. Use the hashtag #NIAM and find resources to share with families in AAP's toolkit here.
-
ICYMI: We’re expanding the conversation to all childhood immunizations: The COVID vaccine is just as important as all other routine childhood vaccinations. When sharing posts consider mentioning how keeping up with all routine vaccinations keeps us all healthy, including COVID vaccination. Help us continue to spread the word about the importance of all routine vaccination for children.
-
Mark Your Calendar: AAP is leading a Vaccine Confidence Day of Action on August 21st on social media to help combat vaccine misinformation and encourage families to make sure their children are up-to-date on their routine childhood, adolescent and COVID-19 vaccines. Help us spread the word about why pediatricians recommend routine vaccinations to all eligible children and families! Be on the lookout for more information and details soon.

Identification of Challenges and Facilitators to Behavioral Health Referral Completion in Children Screened Positive for Adverse Childhood Experiences
Iesha Ticknor (Kaiser Permanente Bernard J. Tyson School of Medicine), Mercie Digangi, DO (Southern California Permanente Medical Group), Sonya Negriff, PhD (Department of Research & Evaluation, Kaiser Permanente Southern California)
Background
In February 2020, Kaiser Permanente Southern California (KPSC) piloted a new workflow to screen for Adverse Childhood Experiences (ACEs) in pediatric primary care. After implementation, only half children referred to behavioral health had completed the referral within 90 days. This qualitative study looked into what parts of the process were making it difficult to complete these referrals. Read more...

A Powerful New Tool to Prevent Severe RSV in All Infants is Now Approved and Recommended
Nirsevimab, trade name BeyfortusTM, is a long-acting monoclonal antibody product, which has
been shown to reduce the risk of both hospitalizations and healthcare visits for respiratory syncytial virus (RSV). It provides critical protection during a baby’s first RSV season, when they’re most at risk for severe illness. Nirsevimab was approved last month by the U.S. Food and Drug Administration (FDA) and earlier this month the CDC recommended one dose of nirsevimab for ALL infants younger than 8 months, born during – or entering – their first RSV season. Additionally, children 8-19months of age who are at increased risk of severe RSV disease and entering their second RSV season should receive one dose of nirsevimab as well. It can be co-administered with other vaccines, and every dose administered should be entered into the California Immunization Registry. CDC is currently working to make nirsevimab available through the Vaccines for Children program for the fall. Additional clinical guidance and healthcare provider education material will be provided in the coming months.

Compliance: The New Immunization Registry Requirement
|
|
New Registry Requirement to Submit Immunization and TB Data
AB 1797, a new California bill effective January 1, 2023, requires providers to enter immunizations and TB tests they administer, as well as a patient’s race and ethnicity, into a California immunization registry (CAIR OR RIDE/Healthy Futures).
This reporting requirement includes newly recommended immunizations, such as
- Nirsevimab to prevent severe RSV disease for infants and toddlers
- RSV vaccines for adults 60 years and older.
Joining CAIR helps providers meet requirements for AB 1797, Medi-Cal, and the Vaccines for Children (VFC) program. There are many other benefits to participating in an immunization registry: save time, improve patient care, boost immunization rates, and quickly run vaccine reports to access data for reporting.
