Pharmacy Benefits Manager Change
Effective January 1, 2025, QualChoice has transitioned our Pharmacy Benefit Manager (PBM). Express Scripts (ESI) will be the new PBM for QualChoice members. Members will receive notification of any impact of formulary changes and may reach out to you to consider possible formulary alternatives provided. Prior Authorizations may be requested starting 1/1/2025 from ESI by any of the methods below. A general Prior Authorization (PA) form and a copy of the 2025 Formulary may be found at www.qualchoice.com on the Pharmacy page.
