BoldAge PACE is seeking Primary Care Physicians with geriatric experience to join our growing, multi-state clinical team. Our value-based model gives physicians the time and support to focus on outcomes and relationships—not volume.

Practice in a True Team-Based Environment 

BoldAge PACE physicians lead care delivery while collaborating closely with an interdisciplinary team that includes:

• Behavioral health specialists 

This model allows providers to deliver comprehensive care while maintaining a manageable workload. 

Multi-State Opportunity 

We’re looking for physicians who: 

• Are licensed (or willing to become licensed) in multiple BoldAge PACE states 
• Reside in one of our operating states: CA, IL, IN, KY, NJ, OH, SC, FL or TN (coming soon). 
• Are open to supporting care across markets (preferred)
