Our "Transitions" program is a facility-based ADT focused on choice-making through personal empowerment. Transitions provides the opportunity for participants to increase self-direction by planning and choosing new experiences each day. We offer a wide variety of interactive classes and community engagement that includes activities in self-advocacy, recreation, fine arts, relationship building, conflict resolution, wellness, current events and much more.
