We look forward to seeing you this week for your upcoming visit.  As you may know, we recently joined Catalyst Physician Group, and committed to keeping you informed on changes to our technology or systems.


Beginning June 29, 2024, we’ve moved our Village Health Partners athena patient portal to a Catalyst Physician Group athena patient portal. This move will deliver coordinated care across all Catalyst Physician Group practices and providers as well as personalized communication and access as we co-manage your health journey. 


We recognize that when we moved to athena last year there was some frustration and disruption, but please know that we have put policies in place to make this a smoother transition for you. 

What does this mean for you?

 

  • When you arrive for your appointment, you will be asked to complete several forms related to HIPAA, PHI and our policies. This will be easy to complete in the existing time allotted for your visit.
  • Please register for the Catalyst Physician Group patient portal before you arrive for your visit:
  • To register: Go to https://30724-1.portal.athenahealth.com/ and create a new account for our CPG patient portal using your existing email address on file. 
  • You will not need to create a new password
  • For any outstanding balances prior to June 29, 2024, please login to https://28398-1.portal.athenahealth.com/

Once registered with the Catalyst Physician Group athena patient portal, you’ll be able to communicate with your provider, check new test results, request prescription refills, view your health history from the existing patient portal and more.
