PopHealth Analytics: Prevention Quality Indicator (PQI) Dashboard 

The Prevention Quality Indicators (PQI) reports are designed to help healthcare providers identify admissions that were potentially avoidable with proper outpatient care. This report highlights areas where beneficiaries may lack adequate outpatient care, require follow-up after hospital discharges, or reveal unmet needs within the community. Additionally, within the reports, users can select from various PQIs (including composite measures). The PopHealth Analytics reports and features can be accessed through the CRISP DC HIE Portal https://idp.crisphealth.org


Key Reports and Insights:

PQI Trend - PQI 92 Report: Tracks the trend of admissions for PQI 92. PQI92: Prevention Quality Chronic Composite - This measure includes adult hospital admissions for diabetes with short-term complications (PQI 01), diabetes with long-term complications (PQI 03), uncontrolled diabetes without complications (PQI 14), diabetes with lower extremity amputation (PQI 16), chronic obstructive pulmonary disease and asthma, hypertension, or heart failure without a cardiac procedure (PQI 05, PQI 07, PQI 08). Note: A lower count indicates better performance. 

  • Comparison of Annual PQI Count Report: Compares the annual count of PQI admissions across different fiscal years. 
  • Count of PQI per Beneficiary Report: Offers insights into the frequency of PQI admissions per beneficiary within a selected timeframe. 
  • Count of PQI by Hospital Report: Provides insights into the distribution of PQI admissions across different hospitals within a selected timeframe. 
  • Count of PQI by APR DRG Report: Offers insights into the distribution of PQI admissions across different All Patient Refined Diagnosis Related Groups (APR DRGs). 
  • Count of PQI by Age and Gender Category Reports: Provides insights into the distribution of PQI admissions across different age and gender categories within a selected timeframe.
