Share of Cost (SOC) Issues
Any other situation in which a refund is due to the plan
All Provider Manuals have been updated with this new P.O. Box information. It's important to note that if you have a refund, the check should be made payable to the applicable plan, i.e., Mercy Care Plan, Mercy Care Advantage or Mercy Maricopa Integrated Care.
