Due to several mail issues experienced during the past year and inconsistencies noted in mailing addresses, refunds for all plans must be sent to the following P.O. Box, effective immediately:

Refunds may be necessary for the following reasons:
Refund payments
Negative balances
Share of Cost (SOC) Issues
Rebate payments
Any other situation in which a refund is due to the plan
All Provider Manuals have been updated with this new P.O. Box information. It's important to note that if you have a refund, the check should be made payable to the applicable plan, i.e., Mercy Care Plan, Mercy Care Advantage or Mercy Maricopa Integrated Care.
