Building Connected Care: Progress on the Canadian Patient Summary

This session will examine the pan‑Canadian Patient Summary as a concise, standardized snapshot of a person’s most essential health information, designed to support continuity of care across settings. Building on the International Patient Summary and adapted through the pan‑Canadian specification, it will explore Canada’s two complementary access models: patient‑mediated access that enables individuals to securely view and share their information, and provider‑mediated access that integrates trusted summary data directly into clinical workflows. The speaker, Sonia Balgah from Canada Health Infoway, will highlight how these approaches together are strengthening care teams, improving information flow at the point of care, and advancing more connected, patient‑centered care.
