Infection Prevention and Control (IPAC) Survey

Please complete this survey (one per office) to identify Infection Prevention and Control (IPAC) needs at your office. This will enable us to develop a community support program to help you and your staff learn about and implement IPAC strategies and procedures in your office/clinical setting.

Please forward the link to the most appropriate person in your office to identify your existing IPAC assets and needs. The survey will close on Wednesday, August 5, 2020.

Mask Exemption Note Requests
Many patients are asking for notes to exempt them from wearing a mask. Here are some resources to help you respond to these requests.
