ChiroCare network provider tier assignment for 2025 will be effective 1/1/2025 which may impact your prior authorization requirements. Prior authorization requirements can be found on the Fulcrum provider portal at www.fulcrumproviderportal.com.

ChiroCare providers will be receiving a tiering letter by USPS mail that will inform you of your 2025 tier assignment no later than 12/1/2024.

Some network providers have mentioned they have received multiple fax notifications in regard to authorization requests. To help ease administrative burden, QConnect correspondence delivery method will be changing to email delivery instead of fax notifications.

Please ensure your email address used to log in to Fulcrum's Provider Portal is active and monitored for authorization related communications.

The change from fax notifications to email notifications will occur no later than 01/1/2025. You will still receive a copy of your authorization outcome letter through USPS mail.

Check your email for a link that will be sent again this Monday to complete the online survey regarding your interactions with Fulcrum Health. This will be the final email. If you have not taken the survey, please do so. A paper survey will not be mailed out to you.

The federal No Surprises Act (H.R. 133, P.L. 116-260) requires providers and health plans to verify the accuracy of provider contact information every 90 days.

Complete the following steps every 90 days to attest, which fulfills the No Surprises Act requirement: 

Step 1: 

View/Update Demographic Information on Fulcrum's Provider Portal.

• Click on the location that you would like to view/update.
• Enter your updated information along with the effective date and then click the Submit button for your changes to be saved.

Step 2:

Attest

• Click on the NSA Attestation button and check the checkbox after reading the attestation and click Submit button.
