Horizon BCBSNJ is always looking for ways to make doing business with us easier and more convenient. That’s why Horizon has changed how they're distributing Form 1095-B to their fully insured subscribers this year.
What Is Form 1095-B?
Form 1095-B reports the names, addresses, and Social Security Numbers of individuals covered under a fully insured health plan and the number of months each member had coverage from January 1 to December 31 of each calendar year. You can learn more about Form 1095-B here.
How Is the Distribution Changing This Year?
In past years, Form 1095-B was mailed to all subscribers enrolled in a Horizon fully insured plan. This year, any subscriber enrolled in a fully insured health plan who registered for a secure member account via HorizonBlue.com will receive an email the week of January 27, 2025, telling them how to access their Form 1095-B online. These subscribers will not receive a printed copy of the form in the mail.
Any subscriber for whom Horizon doesn’t have an email address will receive a printed copy of the form in the mail.
How Do My Clients Access Their Form 1095-B Online?
To view or download Form 1095-B, subscribers must first sign in to HorizonBlue.com, then:

On the left navigation, click on Document Center.

Then click on Tax Documents.
