Long-Term Care Medication Management of Blood Thinners 

Series description: 

Data reveals the number one adverse drug event (ADE) leading to hospitalizations and emergency department (ED) visits in nursing homes is related to anticoagulant/blood thinner medications. Join this Project ECHO® series to improve the quality of care and safety your residents receive, avoid regulatory tags and improve your star quality ratings. Together, we can share tactics addressing these common concerns.

Participants will receive resources to impact rehospitalization, emergency department transfers and fall rates. This series will focus on strengthening Quality Assurance and Performance Improvement (QAPI) processes for enhancing medication management and collaboration efforts with your team, prescribers, consultant pharmacist, residents and representatives.

Join us over your lunch hour to accomplish the following objectives:

• Define the impacts of ADEs in LTC
• Describe the purpose of Telligen’s Anticoagulant Safety Review Tool
• Discuss communication techniques to use when talking with families, residents, prescribers and pharmacists about anticoagulant/blood thinner medications

Audience:

Long-term care administrators and other leaders, Directors of Nursing, nurses, Certified Medication Aides (CMAs), Certified Nursing Assistants (CNAs) and all staff involved in the medication management process who are eager to improve resident safety by decreasing adverse drug events from anticoagulants (blood thinners).

Participants will receive a certificate of completion after attending all sessions.
