Dynamic PDF forms require the use of Adobe Acrobat Reader. Before working with a PDF form, please make sure your computer has a recent version of Adobe Reader installed. (The latest version of the software is called Adobe Reader DC. To download and install Acrobat Reader DC for free, visit the Adobe Acrobat Reader DC webpage.)

The Provost's office has complete detailed instructions and troubleshooting for downloading, completing, and signing Dynamic PDF forms, including what to do if you encounter the Please Wait...message screen. If you are unable to resolve your problem with completing or digitally signing your dynamic PDF form, complete the form "Request for Assistance with Dynamic PDF Forms" located in the troubleshooting section under Other Issue.
