Dear Home Centered,

On behalf of the Home Centered Health Care Institute (HCCI), I am writing today to shine a light on how we’re making a significant difference in the lives of patients and families facing barriers to accessing life-changing home-based primary care.

I am thrilled to share with you some of our 2024 initiatives that will expand through 2025 to help us realize our goal of #HouseCallsForAll: 

• People: I’m most proud of the tireless and selfless work of the HCCI team. More importantly, I’m proud of the practices and patients’ success stories. We are making a difference in people’s lives.
• Partners: We formed new HCCI Connect™ partnerships, like the one with CareSMS. Our HCCI Connect™ platform furthers our cause as it bridges the gap between home-centered care practices and providers and the products, services, and associations they need to succeed. It also provides an avenue for organizations to support HCCI’s mission through sponsorship.
• Programs: On January 22, 2025, HCCI is hosting a webinar about the Centers for Medicare and Medicaid Services (CMS) Final Ruling for HCCIntelligence™ Community Members, as one of many initiatives to help everyone through these changes; join here, and do it now—before the membership price increases on Jan. 1.

There is still so much important work to be done, and we would greatly appreciate your support.
