Unusual Incidents and Major Unusual Incidents: Monthly and Annual Mandates

Unusual Incident Log
Each agency provider and independent provider shall maintain a log of all unusual incidents, to be reviewed by the provider no less than monthly. The log shall contain only unusual incidents and shall include, but is not limited to, the name of the individual, a brief description of the unusual incident, any injuries, time, date, location, cause and contributing factors, and preventive measures. The unusual incident reports, logs, documentation of identified trends and patterns, and
corrective action shall be made available to the county board upon request. You are required complete and send this log to the County Board upon request even if you do not have any UIs to report.

Annual Analysis of MUI Trends and Patterns
By January 31st of each year, all providers shall conduct an in-depth review and analysis of trends and patterns of major unusual incidents occurring during the preceding calendar year and compile an annual report. Providers shall send the annual report to the county board for all programs operated in the county by February 28th of each year. Annual MUI reports for Delaware County can be sent to [email protected]. You are required complete and send this analysis to the County Board even if you do not have any MUIs to report.

Independent Providers - Homemaker/Personal Care, 1:1 = $5.18/unit
Agency Providers - Homemaker/Personal Care, 1:1 = $5.86
