Verify Information in the Provider Directory

Dear Community Partners:

 

The Provider Directory is now LIVE! If you have not yet submitted your record, please verify your information promptly. The steps are as follows: 

  1. Search your email inbox for an email invitation from caddsprod@servicenowservices.com • If you don't see it there, try looking in your junk or spam folder as well. • If you still can't find the email, there may be another issue preventing its delivery. Email ProviderDirectory@dds.ca.gov with the subject "No Registration Link". We will record your email address provided.
  2. Click on the link in the email invitation and register for the Provider Directory.   • Your username is listed in your email invitation and is the email that applies to your registration.  • The link in the email is unique, created just for your account.
  3. Log into the provider directory at https://caddsprod.servicenowservices.com/spd  
  4. Verify that the information populated into Provider Directory from existing DDS sources is accurate.  
  5. When necessary, update information and submit changes to be approved by regional center staff.  
  6. Optional: Save a record of your submission. See section 2.1 of the service provider training guide for instructions on viewing and exporting your record.  



Submit by November 29th for QIP Incentive Payments

DDS will offer incentive payments to service providers who review and submit their information by November 29, 2024, through the Quality Incentive Program (QIP).  For service providers included in rate reform, participation in the Provider Directory measures is the only way to attain the up to 10 percent quality incentive portion of the reimbursement rate from January 1, 2025, through June 30, 2026.  Additional information on the incentives can be found in the Department directive

  • Vendor Name 
  • All vendor numbers for your organization  
  • Email and Contact Person
