We're pleased to announce that Community Bridges, Inc. will assume operational responsibility for Chicanos Por La Causa's Maryvale Assertive Community Treatment (ACT) team, effective April 1, 2017.
This transition is administrative and will not affect services. Members on the ACT team have received a letter communicating these changes and the two agencies will be working together closely to ensure that the transition of care is smooth for the members and those invested in their care.

e appreciate the work and care provided to the members of the Maryvale ACT team under Chicanos Por La Causa (CPLC), and we look forward to our continued partnership with CPLC. CPLC continues to operate the Centro Esperanza ACT team and direct care clinic at their Mesa location.
Members will be transitioning to the new Community Bridges ACT team at CBI's Avondale location, 824 N. 99th Ave., Suite 108, in Avondale. The 99th Avenue team is the second ACT team at the Avondale site, and it has openings for new members. Referrals can be made by calling 602-540-1271. Members will benefit from a primary care provider located on site and a full array of health services from a team that is aware of their whole health needs and works together to improve health outcomes.
CBI also operates three Forensic ACT teams, the Community Emergency Psychiatric Center in Mesa, and the Access Point and Transition Point in Avondale. CBI provides comprehensive, medically integrated behavioral health programs in seven Arizona counties. Its programs include emergency crisis, stabilization and recovery, peer support and outreach, women and children's services, outpatient medical detox and behavioral health treatment, community education and prevention.
ACT teams provide the highest-need members with a full array of community-based supports and services delivered in a wrap-around model so they can achieve their recovery goals and remain in the community. There are 24 ACT teams across the region available to serve 2,400 members. Team members have experience in psychiatry, nursing, social work, rehabilitation services, substance-abuse intervention, housing and employment support, and independent-living skills. Key members of the team are also peers who have lived experience with behavioral health challenges.
The team helps members follow their treatment plans and assists with activities of daily living, employment-related services, finding and maintaining affordable housing, budgeting, obtaining benefits and engaging in community activities, delivering services in accordance with Substance Abuse and Mental Health Services Administration (SAMHSA) evidence-based practices.
Mercy Maricopa continues to provide education and technical assistance to ACT team providers. Together, our goal is to improve ways to connect members to services while following SAMHSA guidelines to:
- Identify and engage members with complex, persistent needs
- Remove barriers to community-based services and supports
- Address the whole person, mind and body
- Reduce hospitalizations
- Reduce contact with the criminal justice system
- Improve health outcomes
- Establish stable housing and employment
- Encourage and strengthen natural community supports
