Provider Directory Update
Our provider directories and online search tool are the most important tools available to our more than 1.4 million members when choosing a provider. We want to help our members, your potential patients, find you quickly and easily through the search tool and directories, which are available on our website at and Delta Dental Plan Association's website at

We are working on updating our provider directory with the most up-to-date information on the providers who are currently practicing at your location, and we need your help! Please click here to download the short form  that needs to be completed with the billing Tax ID Number (TIN) and names of all the providers who are currently practicing at your location.

Once completed, please email a copy of the form to or fax to 303-741-2230 by August 31, 2019.
Provider Portal Log In Workaround
We've been alerted about some provider offices having difficulties logging in to the secure provider portal at We're continuing to work on resolving this matter as soon as possible.

In the meantime, if you're experiencing difficulties logging in, follow the simple steps below:

  3. Click Having trouble logging in?
  4. Enter Username and Password and click Login

Child Health Plan Plus has Transitioned to DentaQuest

Childhood caries steadily declines nationally among American Indians/Alaska Natives, July 1, 2019
Early childhood caries has steadily declined nationally since 2010, and untreated early childhood caries in American Indian/Alaska Native preschool children has "significantly" declined, according to a data brief released June 21 by the Indian Health Service Division of Oral Health.
