Risk/Benefit Discussion Guide
Please remember that SSAs need to complete the Risk/Benefit Discussion Guide and the Team Acknowledgement Form with the team before someone can return to day programming. If you are having any difficulty reaching an SSA, please contact Joelyn Karlson, SSA Director, or one of the SSA Managers directly to address your situation..
Ohio EMA Assistance Funding
The Ohio Emergency Management Agency (EMA) is accepting applications for grant funding under the existing FEMA Public Assistance (PA) Program to respond to and recover from the COVID-19 disaster declaration. 
Provider Assurance Forms
As a reminder, you are required to submit your Provider Assurance forms to both the Ohio DODD and the Medina County Board of DD.
NMT Questions
There a many questions about guidelines for NMT. Unfortunately we continue to hear nothing regarding this matter. When any information becomes available, we will share with you.
Reopening of Services
We know many providers are opening up day programs and may be encountering things they might not have expected. We are here to help if you need any assistance.

As programs and services restart, we can all learn from each other to make sure the individuals we help are able to maintain health and safety. Please share with us any helpful strategies you are using which are working or situations to avoid so that we are able to share with others as needed.

We also continue to have an inventory of some PPE items, cleaning supplies and we are able to purchase other additional items that you may need to address pandemic related issues beyond these items.

As individuals begin to venture out to attend programs, please remember Covid-19 is still present in our community and will continue to spread. Now is a good time to review your plans for preventing and preparing in case either a staff member or individual should test positive for Covid-19. If you need help in either of these areas, please contact our Provider Support Helpline.
