Provider Payment


To ensure timely processing of provider payments, providers must sign and submit completed reports electronically (to include upload of x-rays) through QTC Provider Portal within 48hours of the conducted appointment.

Telehealth Appointments

Providers MUST NOT conduct appointments prior to a Service members scheduled appointment time.

Reminders:

  • Ensure to verbally confirm SM physical location before starting appointment.
  • Confirm you are licensed in the state where SM is currently located, if discrepancy is noted please politely end appointment and call QTC immediately.
  • If the SM does not answer, please make reasonable attempts to reach out to them that day if schedule permits. If you are unable to make positive contact, mark SM as NO Show in provider portal.
  • Appointments should be conducted via phone call and never through text message.

Important: Under no circumstance should a report be submitted if SM was never contacted on the day and time of appointment.

Lab Diagnostics


It is important to note that there are different processes for HIV, DNA, and Quest lab specimen collections and shipment preparation.

  • HIV and DNA labels will be provided through QTC warehouse.
  • All additional lab labels are generated via provider portal for Quest diagnostics.


HIV reminders:

Follow the steps outlined in the one-page reference guide below to prepare labels, collect samples, document labels, prepare and ship the HIV samples.


Effective immediately: Barcodes will no longer be preassigned to a specific service member. We will no longer be using this process and will revert to barcodes being assigned at time of appointment and lab draw. This change in workflow will decrease the rejection rate from Centers for Disease Detection Laboratory (CDD).

HIV Instructions

Quest lab requisitions and specimen labels:


Steps to produce the Quest lab requisition forms and the specimen labels are crucial to avoid them not being properly generated. Please review and follow the step-by-step instructions provided below to guide you and your staff through the correct process.  Labs will not be processed if they are not properly ordered and labeled via our Provider Portal. Your compliance with this process is critical to fulfilling the laboratory studies for service members.

Comprehensive Eye Exams


When an eye exam with refraction and prescription and/or a comprehensive eye exam is ordered, providers must select the "ADD DD771" button at the bottom of the vision screening form. Both vision screening and DD771 forms are to be completed, along with any other services that are noted on the 201 service bill.


Audiogram/DD2216


The medical professional that is conducting an audiogram must be CAOHC certified or an Audiologist. The actual name and certification number of the examiner conducting the audiogram must be recorded in DD Form 2216.

When conducting an audiogram, the current threshold levels are to be entered with numerical values ONLY. If the SM cannot detect the tone at the limits of the equipment this must be noted as "NR" or "100+".

If the provider or technician is unable to conduct an audiogram, it is crucial that QTC is contacted so that the SM can be rescheduled.

Reminder: Conduct a daily functional and biological check of each audiometric equipment prior to use.
