Provider Payment

To ensure timely processing of provider payments, providers must sign and submit completed reports electronically (to include upload of x-rays) through our Provider Portal within 48 hours of the conducted appointment.

Service Member Surveys

Following each appointment, ALL Service Members will receive a brief survey that is aimed to gather feedback, opinions and experiences. This helps in understanding customer satisfaction, improving services and identifying any areas for growth or change.

We offer the following tips:

  • Approach all Service Members in a considerate manner.
  • Avoid interruptions and delays in initiating the exam.
  • Avoid making jokes, inappropriate remarks or personal comments.

Physical Examinations

Comprehensive Physical Exams, also referred to as 'A-Physicals' are head to toe examinations that are conducted to ensure the medical qualification of Service Members based on Service Component guidance. A-Physicals should not be confused with Periodic Health Assessments (DD3024)

A-Physicals for RHRP are medical examinations conducted to ensure Service Members meet the medical and readiness standards required for their military duties. A-Physicals are typically conducted for:

  1. Military separation
  2. Retirement
  3. Retention
  4. Commission
  5. Accession

These exams require:

  • Comprehensive Medical Review: Evaluation of Service Members overall health, including review of medical history and current health status (DD2807)

  • Physical Assessment: Hands on examination of body systems, such as cardiovascular, respiratory, musculoskeletal, and neurological. (DD2808)

Click the links below for sample DD2808 forms with highlighted requirements:

  • Vitals and Exam Specific Tests: Measurement of height, weight, and blood pressure are taken at each visit as well as any required labs. Depending on age, gender, and military requirements, screenings for vision, hearing and other specific conditions may be included.
