Providers can now unlock Service member questionnaire if SM has changed their responses since completing the form on service member portal.




PPE and Lab Standard Precautions

Personal protective equipment and standard precautions must be used at all times when there is a potential risk of handling bodily fluids. Please refer to the PPE and Lab Standard precaution guidelines.


Status of Upload

From the provider portal appointment page. After a file is uploaded, users will see the status:

Green check mark= successfully uploaded

Red X= failed upload

Grey circle with line= In process of uploading

Provider Complete Status


Cases with Appointment status of Provider Complete will only disappear/drop from your Provider Portal when the service member has completed ALL required appointments with Leidos QTC. Therefore, you may see cases remain on your provider portal even for some time after you have completed your report through to Provider Complete status.

2813 check box for 603A


The 2813 check box at the top of the 603A dental assessment form should not be selected if service members are being seen in a dental clinic setting. This selection option is only to be utilized at RHRP group events.

2807/2808 Linking Demographics


The service members demographic information within the Report of Medical History form (DD2807) and Report of Medical Examination form (DD2808) will now be linked. Therefore, providers and staff will no longer have to enter the following demographic data within the DD2808 form.

Linked Fields include:

  • SM Full Name
  • SSN
  • Today's Date
  • Home Address
  • Home Telephone
  • Email Adress
  • Name of Examining Location and Adress
  • Service Component
  • Purpose of Examination
  • Grade/Rank
  • Sex

Quest Lab Ordering



All Lab orders except for HIV and DNA testing must be sent to Quest diagnostics. If labs are ordered for an appointment, an icon will appear under the Diagnostics column. Click on the icon to see which labs are ordered and then print the requisition form as well as label.

Service Member Surveys


Following each appointment, ALL service members will receive a brief survey that is aimed to gather feedback, opinions and experiences. This helps in understanding customer satisfaction, improving services and identifying any areas for growth or change.


Approach all service members in a considerate manner. Avoid interruptions and delays in initiating the exam as well as making jokes, inappropriate remarks or personal comments.
