Provider Reimbursement


QTC IS THE SOLE PAYOR. PLEASE DO NOT BILL THE SERVICE MEMBER UNDER ANY CIRCUMSTANCES.



Providers do not need to bill QTC separately. Payment will be processed through QTCs payment process based on reports submitted through QTC Provider Portal. For providers that are associated with business partners, please reach out to your point of contact for more reimbursement information.

Post Deployment Health Re-Assessment (Service Member Section)


Often times the Service Member's questions are either partially answered or completely blank. There are numerous reasons for this, including any technical issues the Service Member may have encountered while trying to answer the questions.


While it is not a requirement for the Service Member to have these questions answered prior to their appointment, we do encourage them to do so.


Please assist, when necessary, and do not turn the Service Member away or request to reschedule should their section not be entirely completed.

Form Update

All cases created AFTER May 23rd will have this update on the Limited Duty Recommendation Form.

This additional form is required by DoD when a Service Member (SM) has a Duty Limiting Medical Condition, indicated YES from PHA Part C.

CHANGE: PULHES data fields have been removed. For cases prior to this release, continue to select the PUHLES N/A checkbox if unable to complete.

Unusual Incident


Unusual Incident is defined as something that occurred during provision of service that may have or will result in:

  • risks to health and safety
  • result in legal claims
  • involvement of senior Government officials


An Unusual Incident Report (UIR) must be completed for any of the above scenarios, including ALL Suicidal and Homicidal ideation risk levels - high, intermediate and low.

Whom to contact in the event of an Unusual Incident (Emergent):


  • In-person or telephone appointment: Provider and/or clinic staff contacts the QTC Customer Service Agent, who then notifies the military unit POC. The provider then completes an emergent referral and Unusual Incident Report via the Provider Portal.
  • Group Events: Provider and/or clinical staff takes the Service Member through the appropriate Chain of Command, who then takes control of the situation. The provider then completes an emergent referral and Unusual Incident Report via the Provider Portal.
