2023 Anthem Quality Program Scorecards


Thank you all for your continued contributions this year as we work towards reaching our goals for the 2023 Anthem Quality Program. Month over month, we are making steady progress as a network- let's all give each other a well-deserved high-five for our efforts!

We have updated our Anthem program flyer to reflect our current network performance. Please also click the updated scorecard links below to view your individual practice's performance. Remember, the goal is to monitor our ongoing progress and implement small improvements that can lead to big results!

As an added reference tool, we are also sharing the measure scorecard for Chlamydia Screening. Although this is not currently a measure of focus, we realize it is an important measure that many practices may want to evaluate their performance with their peers.


As a network we aim to improve all of these measures of focus listed above. To do this we need your help. Please review each scorecard linked above. A secret number is listed for your practice. If you do not recall your practice’s secret number, please reach out to Liz Hogan at Elizabeth.hogan@tccn-choa.org for assistance.


As promised, we have also included our updated measure definition sheets, which outlines the parameters and coding requirements of each measure included in the Anthem Quality Program along with additional resources to help you. These resources have been reviewed by specialists in the area to ensure we have the most up-to-date information and guidelines to share with you.

Quality Programs

TCCN Minimum Standard Scorecards

TCCN is committed to Network Quality Improvement and the ability to demonstrate change in practice. As we continue to improve and evolve as a network, the setting of minimum standards is an important step to take in order to ensure compliance and meet Clinical Integration Requirements. 


For this year, we will continue to place a focus on Asthma Action Plan, BMI coding, and depression screening as important measures of focus.


As a reminder, the minimum standards for 2023 are:

 

  • All practices are required to meet the minimum standard of 15% for coding for Asthma Action Plans by 12/31/2023.
  • All practices are required to meet the minimum standard of 55% for coding for BMI by 12/31/2023.
  • All practices are required to meet the minimum standard of 45% for coding for Depression Screening by 12/31/2023.


Although the well child 3-21 metric is not currently a component of our minimum standard program; it is a measure of focus for this year and we are including it so that we can continue to monitor our progress on this important measure as a network.

Asthma Flu Recall Lists

One thing we can do to prepare for flu season is to start recall efforts early so that we can start conversations with patients about the importance of getting their flu vaccine. The flu vaccine continues to be one of the best ways to help provide the greatest protection possible against the flu and other communicable diseases, especially for vulnerable populations including asthmatics.


To assist you in your recall efforts, TCCN will be sending your practice a list of Asthmatic patients that are due for the flu vaccine. Please use this list as a helpful tool in your patient communication efforts. This list is pulled from our population health management tool, Innovaccer, and is up to date as of the last time you sent claims data.


Please be on the lookout! TCCN will be sending out recall lists in early September.

DEI-Focused Clinical Quality Forum Recording


Thank you to everyone who attended our virtual Clinical Quality Forum. We had 140 attendees, which is one of our highest turnouts and a great start to our programming for the year!

 

If you were not able to attend live, we highly encourage you to watch the recording and complete the attestation via this link or the QR code to receive your TCCN participation credit.


The recording is roughly 1 hour. If you attended live, there is no further action needed for your practice to receive credit. At least one provider from each practice must view this program or the upcoming fall educational program. In addition, each practice must view the short DEI educational video that was at the beginning of the module, so your participation for this program will fulfill two of your 2023 requirements.
