Dear Students:
Thank you to those who have completed the Student Vaccination Mid-Year Questionnaire.
As a reminder, all students engaging in in-person activities sponsored by ESF are required to get a booster shot within 30 days of becoming eligible.
If you have not yet verified your status by completing the questionnaire, we strongly urge you to do so as soon as possible to ensure that your record is current, you continue to have access to campus services at ESF and Syracuse University, and to avoid unnecessary quarantining. If your booster status has changed since you previously completed the questionnaire, please provide an updated response here: Vaccine Booster Update.
If you have not yet provided your vaccination or booster status and are identified as a close contact of a person who tests positive for COVID-19, you have a greatly increased risk of being issued a quarantine order by the New York State Department of Health. Help reduce the chance that you are quarantined by immediately providing the information in the ESF’s questionnaire.
Here are the steps to take:

Enable the DUO app (this allows for access into the College’s systems if you are off campus). Refer to this link for instructions on how to set this up.
- Clear your cache to be sure your credentials aren’t still there. The Microsoft 365 environment will capture whatever credentials you last used even if you open a new private browser.

Open a new browser in private/incognito mode and use your ESF.EDU credentials to log in (not your SYR.EDU credentials).
