As part of being able for students to return to campus in Phase 2 in the future, and specifically in order to obtain the elementary waiver for on-campus instruction for grades K - 5 right now, RCS Employees must begin routine COVID-19 testing.
Per the Alameda County School Reopening Plan, "The public health guidance recommends staff in every California school be tested for COVID-19 periodically based on local disease trends and as testing capacity allows." RCS needs for approximately 50% of employees to be tested each month to meet the guideline. This will be accomplished on the honor system. If you don't sign up this month, then you will need to sign up next month.
Therefore, RCS has scheduled phlebotomists from PMH Labs to conduct our first routine COVID-19 testing on the elementary campus on Wednesday, 9/23 between 8 - 3:30 pm, in N12 at the District Office. One of the benefits of using PHM Labs is that the testing will be free for employees and free for RCS. PHM Labs will directly bill your health insurance. If you do not have health insurance, please bring with you the Uninsured Patient Form, and the program will pay the fee for you.
Employees will schedule their appointment via the online scheduler (linked below) and receive results within 72 hours through an online portal. You will need to login to get your own results.
The nasal swab test is the test required. You will have a choice of also getting an antibody test which requires one additional form to be filled out (Consent For Venus Puncture).
Testing will take place in N12 at the District Office. Please schedule your appointment during a time when you have a break from your teaching. You will need to bring the following forms to your appointment (forms included in the button below):
- Patient Test Request Form
- Venous Consent Form (if doing the antibody test)
- Employee Authorization Form to Disclose Results (optional)
- A photocopy of the front and back of your medical insurance card or Uninsured Patient Form (and required items for that form)
