Recapping 2024 and looking to 2025!

2024 was a big year for the Collaborative! We are taking a moment to share a timeline of accomplishments from the past 12 months. We look forward to continuing this amazing work with you all and hope to see you at our next all-member meeting on Friday, January 24th at 1 pm MT.


2024 Milestones

Spring 2024

  • Established the NMSDOH-C Charter. The year started with the Collaborative approving a Charter available here. The Charter provides information about the Collaborative's governance structure and defines NMSDOH-C membership.
  • Published the Environmental Scan Report. The report highlights findings from examining current CLRS efforts across New Mexico, engaging the New Mexico Social Determinants of Health Collaborative (NMSDOH-C) in interviews and a landscape assessment survey, and identifying recommendations from states currently implementing CLRS. 


Summer 2024

  • Welcomed a new Steering Committee. Steering Committee Members were nominated by and voted in by members that completed the membership form. Read Steering Committee bios here and sign up for Collaborative membership here.


Fall 2024

  • Hosted our first Learning Circle Convening. The Learning Circle on September 9th was an opportunity for members to meet-in person and to share and build upon collective wisdom and lived experiences. To read the Learning Circle Report click here.


Winter 2024

  • Facilitated a Webinar Series on the following topics selected by Collaborative members:
  • Closed-loop Referral Systems (CLRS) 101 - Watch the recording.
  • Organizing and Coordinating State Efforts to Address Common SDOH - Watch the Recording
  • The Importance of Incorporating SDOH data into Health Information - Watch the recording.
  • Reconnected with HCA to guide CLRS Development and Implementation. Roberto Martinez will be working with the Collaborative to organize feedback sessions on topics related to the design and implementation of the statewide CLRS. More updates regarding these sessions will be available soon.
  • Adopted the Seven Principles of the NMSDOH-C. These principles were developed at the Learning Circle and adopted by the Steering Committee in December. To view the principles, visit our About Us page on the NMSDOH-C site linked here.
  • Created an FAQ document. Collaborative members informed the questions and the answers within this document for new members and those curious about the Collaborative's work. Read the FAQ document here.
