With the project, HCCI invites passionate Illinois providers to apply for selection — including independent practices, those affiliated with health systems, and hospice and palliative care programs looking to add or expand their home-based primary care services. The deadline to apply is Friday, May 3.
Apply now for the opportunity to be selected and receive over $25,000 in benefits:
• Get a Practice “Checkup”: HCCI will examine all aspects of your practice, including billing and coding, geographic scheduling, and staffing. Then we will show you how to use tailored market intelligence from our Confer Analytics™ platform to pinpoint opportunities for growth.
• Receive Comprehensive Training & Resources: You will take part in extensive operational and clinical training and receive numerous resources, including access to select on-demand, web-based courses; customized education activities; and HCCIntelligence™ Premier Tools & Tip Sheets.
• Opportunity to Acquire Capacity-Building Funding: There will be an opportunity to apply for and receive funding to support your practice’s growth and sustainability goals.
• Gain Access to Industry Experts: Receive exclusive access to experts in the field of home-based medical care and community support from your colleagues in the second cohort and previous cohort.
Karly Denes, HCCI’s Outreach and Engagement Specialist, will also support the selected practices all along the way. “I’ll hold a practice’s ‘hand’ throughout the whole process,” Denes said. “We're going to offer many types of support. If, for example, you need assistance setting up a staffing model for offering house calls or billing for a specific service, then I’ll collaborate with you and find resources for whatever challenge you're facing.”
Be part of pioneering a healthcare movement in Illinois. Apply here by May 3.
